Community Support Worker
Vita Community Hub,
Fixed Term Contract -8 months, 25 hours
To assist in the delivery of individualized community support to a number of clients with intellectual, physical and or mental health disability., some who may present with behaviors that challenge Supporting clients in their communities and some in their own home, working towards optimizing their social, psychological, intellectual, domestic community living and daily living functioning .
Minimum Education & Skills required
§ QQI Level 5 in Health & Social Care or equivalent
§ Minimum of one year work experience in the area of intellectual
§ Experience of working 1:1 with clients in the community
§ To support individuals with independent living skills
Job Duties & Responsibilities
§ To plan, develop, implement, monitor and review appropriate educational, recreational and occupational programs within the community ensuring best practice in line with up to date research regarding improving people Quality of Life.
§ To liaise with families, the multidisciplinary team and key community based personnel in the pursuit of accessible community activities for service users.
§ Support the individual to enhance their image by exploring and positively influencing personal presentation, language and where and how the individual spends their time. This should be in line with the individual’s age and culture
§ Full clean driving license
§ To comply with health and safety legislation.
§ To liaise with families, the multidisciplinary team and other personnel as directed by the Community Services Manager
